Compatibilité par évaluation visuelle du thiopental injectable lors de son administration en Y avec des médicaments usuels

Objectifs : En 2011, la compagnie Hospira cessa la commercialisation du thiopental. Toutefois, des tests de compatibilité étaient déjà en cours à notre centre hospitalier, puisqu’il n’existe qu’un nombre limité de données disponibles concernant la compatibilité du thiopental injectable avec d’autres médicaments. L’objectif du projet était de tester la compatibilité du thiopental administré en Y avec plusieurs médicaments fréquemment utilisés aux soins intensifs afin de guider les pharmaciens dans leurs recommandations lors d’administrations intraveineuses.

Méthode : La méthodologie de l’étude est fort simple et comprend le mélange un pour un du thiopental avec successivement plusieurs autres médicaments injectables et l’observation du mélange sur une période n’excédant pas quatre heures.

Résultats : Une incompatibilité visuelle du thiopental avec une grande quantité d’autres médicaments est rapportée. Par ailleurs, parmi les médicaments testés, seuls l’insuline humaine, le lorazépam et le triméthoprim/sulfaméthoxazole sont visuellement compatibles avec le thiopental lors d’une administration en Y.

Conclusion : Les données publiées corroborent nos résultats dans plusieurs cas, démontrant le haut niveau d’incompatibilité du thiopental avec la majorité des autres médicaments testés. Ces données sont publiées dans l’éventualité d’une nouvelle possibilité de disposer de la molécule, en supposant une formulation similaire à celle du produit testé.

Abstract

Objective: In 2011, Hospira stopped marketing thiopental. However, compatibility testing was already underway at our hospital, since there was a limited number data on the compatibility of injectable thiopental with other drugs. The objective of this project was to test the compatibility of thiopental administered via Y-site with a number of drugs frequently used in intensive care, and to guide pharmacists in their recommendations.

Method: The study methodology consisted in mixing, one by one, thiopental with a number of injectable drugs and observing the mixture for a period not exceeding 4 hours.

Results: Visual incompatibility between thiopental and a number of other drugs is reported. Of the drugs tested, only human insulin, lorazepam and trimethoprim/sulfamethoxazole were visually compatible with thiopental during Y-site administration.

Conclusion: In many cases, published data corroborate our results, which show a high level of incompatibility of thiopental with most of the other drugs tested. These data are being published in the event that thiopental becomes available again, assuming a formulation similar to that of the product that was tested.
